San Juan ACO (the “ACO”) is a collaborative organization serving the rural region of Western Colorado, including the communities of Montrose, Delta, Gunnison, and smaller outlying communities in the region. Its participants include three hospitals, their employed physicians, solo and independent practices, a rural health clinic, and Federally Qualified Health Centers (FQHCs). The ACO participants share similar challenges associated with mountainous geography, limited financial and human resources, and navigating the transition from volume- to value-based reimbursement.
The San Juan ACO’s primary purpose for the ACO is to provide its participants with the opportunity to develop the competencies and the capabilities to effectively manage cost and quality. The ACO is undertaking every possible means to achieve efficiencies through collaboration among its multiple medical communities, and through the utilization of CCA’s strong and efficient centralized infrastructure, which has even greater scale. The ACO evolved from their participants’ and provider stakeholders’ mutual interest in pursuing similar initiatives that promote quality care, drive care efficiencies, and engage patients in coordinated care.
