San Juan ACO (the “ACO”) is a new collaborative organization serving the rural region of Western Colorado, including the communities of Montrose, Delta, Gunnison, and smaller outlying communities in the region. Its participants include hospitals, their employed physicians, solo and independent practices, a rural health clinic, and Federally Qualified Health Centers (FQHCs).
All ACO participants share similar challenges associated with rural geography, limited financial and human resources, and navigating the transition from volume- to value-based reimbursement.
The San Juan ACO is governed by a Board of Managers comprised of hospital and physician participants representing the various communities and regions it serves. The primary purpose for the ACO is to provide its participants with the opportunity to develop the competencies and the capabilities to effectively manage cost and quality. The ACO evolved from ACO participants’ and provider stakeholders’ mutual interest in pursuing similar initiatives that promote quality care, drive care efficiencies, and engage patients in coordinated care.
The ACO and its participants are committed to investing time, sharing resources, and affiliating with necessary stakeholders, in order to identify care coordination and efficiency solutions that create clinical best practices and drive down overall costs.
